HLA-DPB1 and HLA Class I Confer Risk of and Protection from Narcolepsy

Abstract: Type 1 narcolepsy, a disorder caused by a lack of hypocretin (orexin), is so strongly associated with human leukocyte antigen (HLA) class II HLA-DQA1(∗)01:02-DQB1(∗)06:02 (DQ0602) that very few non-DQ0602 cases have been reported. A known triggering factor for narcolepsy is pandemic 2009 influenza H1N1, suggesting autoimmunity triggered by upper-airway infections. Additional effects of other HLA-DQ alleles have been reported consistently across multiple ethnic groups. Using over 3,000 case and 10,000 control i… Show more

“…Indeed, T1N is tightly associated with the human leukocyte antigen (HLA) HLA-DQB1*06:02 allele, carried by 98.4% of patients vs. 17.7% of the general European population (4). An independent association with HLA class I alleles was recently revealed in two independent studies (5,6). Additionally, an association with polymorphisms in the T-cell receptor (TCR) α chain locus was found and replicated (7,8).…”

“…Interestingly, two recent studies found additional minor associations with HLA-B*35 or HLA-B*51:01 [21, 73, 74]. These findings suggest that cytotoxic responses mediated through by natural killer or CD8+ T cells are important in the development of narcolepsy.…”

“…HLA- DPB1*05:01 in particular has been shown to be predisposing [21, 73] whereas HLA- DPB1*04:02 is protective [21, 73]. Similarly, to DQ heterodimers, it is possible that these other heterodimers bind peptides that are either mediating response to environmental triggers such as influenza A infection.…”

“…Additional risk alleles in both MHC class I and II have also been identified, which might influence adaptive immune response and virus clearance. The association of HLA alleles with narcolepsy is, thus, more complex than hitherto presumed (11,12).…”
