HMG-CoA Reductase Inhibitors and Myotoxicity

Abstract: The 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitors specifically inhibit HMG-CoA reductase in the liver, thereby inhibiting the biosynthesis of cholesterol. These drugs significantly reduce plasma cholesterol level and long term treatment reduces morbidity and mortality associated with coronary heart disease. The tolerability of these drugs during long term administration is an important issue. Adverse reactions involving skeletal muscle are not uncommon, and sometimes serious adverse reac… Show more

“…Increases in CPK level have been observed with lipid-lowering therapies, particularly high doses of statins or upward dosage titration. 17 Coadministration of ezetimibe with low-dose atorvastatin was a well-tolerated option to high-dose atorvastatin monotherapy.…”

“…The monitoring of CK levels is considered a useful parameter in tracing myotoxicity in patients using anti-lipemics 3,8 , including those who are assymptomatic 5,6 , although there are reports of confirmed toxic myopathy cases with normal muscle biopsy and normal CK 12 . A rise in CK during therapy with statins is well documented, ranging between 0.1% and 10% 6 , usually not exceeding 10 times over the upper limit of normal 3,5 .…”

“…The most severe risk of these drugs is myositis with rhabdomyolysis. Rapid remission of symptoms after discontinuation of therapy has frequently been reported [1][2][3][4][5][6][7][8][9] .…”

“…Apoptosis is a critical mechanism in the remodelling and maintenance of tissue structure and inappropriate apoptosis can produce pathological conditions (121,122) . Some of the decrease in CoQ 10 can probably be explained by the reduction in LDL-cholesterol levels after statin therapy, since CoQ 10 is transported in the LDL particle (121) .…”
