Hobnail Hemangioma (Superficial Hemosiderotic Lymphovascular Malformation) in Children

Abstract: Hobnail hemangioma is an uncommon benign vascular malformation. Due to its benign nature, treatment is not required. If treatment is indicated, complete surgical excision appears to be the most effective option.

“…Hobnail hemangioma (HH), also known as targetoid hemosiderotic hemangioma, is a rare, benign vascular anomaly 1 . Classic presentation is a small, solitary papule or plaque with central red‐to‐violaceous coloration and a peripheral ecchymotic ring on the limbs or trunk in young to middle‐aged adults 2 . However, the cyclical nature of the lesion can lead to fluctuations in morphology, and many lesions lack the characteristic targetoid appearance 2,3 …”

“…119 Others regard the HH as a lymphatic malformation. 120 Spindle cell hemangioma (SCH), once thought to be a low-grade malignant vascular neoplasm with the appellation of spindle cell HE, has been reclassified as a benign entity which presents in children and young adults as a nodule typically in the distal extremities and less common in other sites. 121 Although most cases of SCH are Figure 8.…”

“…It would appear, based on D2-40 immunopositivity, that HH is another lymphatic lesion and possibly should be regarded as a “true lymphangioma.” 119 Others regard the HH as a lymphatic malformation. 120…”
