Hodgkin's Disease Variant of Richter's Syndrome: Experience at a Single Institution

Abstract: Patients developing Hodgkin's disease (HD) after a diagnosis of chronic lymphocytic leukemia (CLL), are frequently included in a series of patients with Richter's syndrome (RS). We sought to determine the natural history of the association of CLL and HD. Over a 21 year period, 1374 patients with CLL have been registered in our computer data base. Seven cases of CLL and HD have been documented and confirmed. The median age of these patients was 71 years (range 44-77) and clinical features included male gender (… Show more

“…In a study involving 1374 cases of CLL at the MD Anderson Hospital (Houston, TX, USA) there were 47 cases of large cell type Richter's syndrome compared to only seven cases of Hodgkin's type of transformation. 3 However, this may be an underestimate since biopsies of lymph nodes or other extranodal sites are not routinely carried out in many patients when the disease is progressing because the features are attributed to progressive CLL.…”

“…4 In the last 8 years however, cases of Richter's syndrome with Hodgkin's-like cells (H-RS) supervening on CLL have been described. [1][2][3] The comparative incidence of the two types of transformation is difficult to determine. In a study involving 1374 cases of CLL at the MD Anderson Hospital (Houston, TX, USA) there were 47 cases of large cell type Richter's syndrome compared to only seven cases of Hodgkin's type of transformation.…”

“…In approximately 3 to 5% of cases of CLL, the disease progresses to a large-cell lymphoma or Hodgkin's disease (2,(5)(6)(7)35). Histological progression to large-cell lymphoma also corresponds to a general clinical alteration, as observed in our patients (1,2,12).…”

“…Approximately 3 to 5% of cases of chronic lymphocytic leukemia (CLL) show evidence of morphologic transformation into diffuse large-cell lymphoma (DLCL; 1, 2) or Hodgkin's disease (3)(4)(5)(6)(7)(8). This process is commonly referred to as Richter's syndrome (RS; 9, 10).…”

“…[46] When multiagent chemotherapy is administered, overall response rates of around 50% and short progression-free intervals have been reported. [20,25] Generally, clinical outcomes are worse than in de novo HL.…”
