Home Health Care Utilization: A Review of the Research for Social Work

Abstract: The author reviewed the literature to identify the variables associated with home health care utilization using the Andersen-Newman model as a framework for analysis. Sixty-four studies published between 1985 and 2000 were identified through PUBMED, Sociofile, and PsycINFO databases. Home health care was defined as in-home skilled nursing, homemaker, mobile meals, home health aide, physical therapy, occupational therapy, or social work services. The review indicates that the client most likely to use home heal… Show more

“…One explanation for the finding may be that other things being equal, more severe cognitive impairment may have prevented from seeking formal help, whereas those with only mild impairment still had this capacity. However, the role of cognitive impairment may have been left uncertain here as also in previous studies; perhaps, due to the inaccuracy of cognitive test scores in measuring the effect of cognitive status on the use of services (Kadushin 2004).…”

“…Other factors include, for example, age, gender, education, income, population density or level of urbanization and access to care (e.g. Branch and Jette 1983;Kemper 1992;Larsson and Thorslund 2002;Kadushin 2004;Van Houtven and Norton 2004;Geerlings et al 2005;Broese van Groenou et al 2006). In addition to the abovementioned determinants, one of the most important factors in predicting use of formal help is receipt of help from informal sources (Bass and Noelker 1987;Kemper 1992).…”

“…Determinants of formal home care utilisation have been examined in numerous studies, predominantly in the United States, but also in Europe (see Kadushin 2004 for a review). The behavioural model proposed by Andersen and colleagues (Aday and Andersen 1974;Andersen 1995Andersen , 2008Andersen and Newman 1973) has become the dominant model in this research field (Calsyn and Winter 2000;Kadushin 2004;Lyons and Zarit 1999 (Larsson 2004;Geerts 2010;Kadushin 2004).…”

“…The present study, by contrast, focused on transition patterns over time (substitution and complementarity, transition to informal and formal care use). The analysis also considered crossnational differences in the impact on the transition to formal care of two factors consistently identified as being strongly associated with home care use: health status and household composition (Kadushin 2004). Whilst various other studies have examined country differences in formal and informal care use and macro-level influences on care utilisation (see for instance Motel-Klingebiel et al 2005), very few have explored how macro-contextual factors might affect the impact of individual-level determinants.…”

“…According to previous studies, the majority of homecare clients are women, rather old, living alone with comorbidity and functional problems (Algera et al 2004;Fortinsky et al 2004;Kadushin 2004;van Campen and Woittiez 2003;Bruce et al 2002;Lee and Mills 2000). The loss of function usually begins with those activities that are the most complex and least basic such as house cleaning (e.g., Dunlop et al 1997).…”
