Background The chikungunya virus (CHIKV) is a re-emerging alphavirus that can cause chronic and potentially incapacitating rheumatic musculoskeletal disorders known as chronic chikungunya arthritis (CCA). We conducted a prospective cohort study of CHIKV-infected subjects during the 2013 chikungunya outbreak in Martinique. The aim of this study was to assess the prevalence of CCA at 12 months and to search for acute phase factors significantly associated with chronicity. Methodology/Principal findings A total of 193 patients who tested positive for CHIKV RNA via qRT-PCR underwent clinical investigations in the acute phase (<21 days), and then 3, 6, and 12 months after inclusion. The Asian lineage was identified as the circulating genotype. A total of 167 participants were classified as either with or without CCA, and were analyzed using logistic regression models. Background Hong Kong is geographically located in the province of Guangdong which, after Hubei, has been the region of China second-most affected by the COVID-19 pandemic. Compared to the pathognomonic symptoms of the named disease, homeopathic symptoms are always more helpful for homeopathic prescriptions. Aim This study reports and summarizes the homeopathic symptoms observed in 18 confirmed/suspected epidemiologically related cases in cluster outbreaks of COVID-19 in Hong Kong in early 2020. Methods Homeopathic symptoms from this case series were collected from 18 consecutive patients who, in addition to their concurrent conventional treatment or traditional Chinese medicine, actively sought help from homeopathy as an adjunctive measure for symptomatic relief from COVID-19. Cases were categorized according to outbreak clusters, focusing mainly on the homeopathic symptoms. In the analysis, frequency of all homeopathic medicines, common rubrics in all the cases, common rubrics in each of the top-ranked remedies, and differentiating symptoms for each top-ranked remedy were determined. Results Homeopathic symptoms of 18 cases, each identified as mild and belonging to one of six separate clusters, are reported. Eighteen common symptoms screened out of 79 selected rubrics constituted two sets of homeopathic symptom pictures: Bryonia alba (n = 4) and Gelsemium sempervirens (n = 12). Eight and seven differentiating features, respectively, were identified for Bryonia alba and Gelsemium sempervirens. Conclusion The common symptoms of 18 mild COVID-19 cases constituted two sets of homeopathic symptom pictures, indicating Bryonia alba or Gelsemium sempervirens; they were indicated in 4 and 12 cases, respectively, out of the 18 in total.

Polyarticular arthritis with swollen tender joints and severe debilitating pains leading to immobilization is a common symptom of Chikungunya fever. In a majority of the patients, the joint pains resolve in 1 to 3 weeks. However, symptoms may persist in about 33% of patients for 4 months, 15% for 20 months and in 12% of the patients for 3-5 years. [1,2,4,5] This persistent joint pain may require analgesic and long-term NSAID therapy which often does not provide adequate pain relief and is associated with serious GI and cardiovascular side effects.[10] Homoeopathic literature mentions a large number of medicines that have proven efficacy for joint pains. [11,12] It is therefore relevant to explore the efficacy of Homoeopathic treatment for post Chikungunya Arthritis.This study reports 20 cases who complained of persistent joint pains with or without swelling after suffering from fever that met the European Centre for Disease Control (ECDC) clinical and epidemiological criteria [3] for Chikungunya fever; and agreed to discontinue analgesics during homoeopathic treatment. VAS scale and musculoskeletal examination criteria [13] were used to assess the efficacy of treatment. The results indicated significant improvement in pain, tenderness, swelling and range of movement. Homoeopathy can be effective for treatment of post chikungunya arthritis which is evident from the p value (P=1). We need randomized controlled trials with a larger sample size and longer follow up duration to conclusively establish these findings.
