Hookworm Folliculitis

Abstract: A case of persistent folliculitis in a 21-year-old man was demonstrated to be due to Ancylostoma caninum larvae. Treatment with oral thiabendazole was curative. Cutaneous larva migrans may be due to A caninum, but this presentation appears to be unique. The literature concerning etiology and pathogenesis of larva migrans is discussed with reference to this case.

“…Pelodera strongyloides is a nematode belonging to the order Rhabditida, which lives in cold areas (latitude 40–60°) and usually affects the skin of dogs, cattle, horses, and sheep. Subsequently, 20 cases of follicular HrCLM have been published (Table ) . Follicular HrCLM occurs especially in adults; only one non‐adult case, occurring in a 6‐month‐old infant, has been described .…”

“…Follicular rupture is rare. 14 Histology of scabetic burrows demonstrates mites, larvae, eggs and excreta in the stratum corneum. 6,12 It has been shown in classic cases of EPF that there are neutrophilic and eosinophilic chemotactic factors within stratum corneum extracts from lesions.…”

“…In fact, unlike A. caninum, A. braziliense does not possess specific collagenase and thus is unable to cross the basement membrane (3) and reach the intestine in order to complete the life cycle. On the other hand, A. caninum larvae migrate into the dermis and enter the circulation, thus causing Loffler' s syndrome, which is characterized by asthma, pulmonary infiltrate, eosinophilia, fever, polmorphous erythema, and occasionally urticaria (2)(3)(4)(5).…”
