Hospital Consultation From Outpatient Clinicians for Medically Complex Children

Abstract: IMPORTANCEChildren with medical complexity (CMC) frequently experience fragmented care. We have demonstrated that outpatient comprehensive care (CC) reduces serious illnesses, hospitalizations, and costs for high-risk CMC.
