How best to approach endocrine evaluation in patients with HIV in the era of combined antiretroviral therapy?

Abstract: SummaryEndocrine and metabolic dysfunction has been documented throughout the history of clinical experience with HIV and AIDS. Opportunistic infections such as CMV and TB adrenalitis, tumours affecting endocrine organs, and cachexia and wasting can still be seen, particulary in severely immunocompromised individuals who may be noncompliant with, resistant to, or without access to effective antiretroviral therapy (ART). However, in those with good control of their HIV infection, the profile of endocrinopathy i… Show more

“…HAART has been linked to increased systemic inflammation and autoimmunity [14,15]. IRIS has been defined as CD4 count > 200 cells/mm 3 in patients who previously had CD4 counts < 100-200 cells/mm 3 [16]. Hence patients in our study with baseline CD4 counts < 200 cells/mm 3 , which increased to > 200 cells/mm 3 at first follow-up following HAART initiation, were defined as having IRIS.…”

“…Immune reconstitution inflammatory syndrome (IRIS) has been defined as an increased CD4 count above 200 cells/μl in patients who previously had <100-200 cells/μl of CD4 counts 12 13 . Hence, patients in our study with baseline CD4 counts <200 cells/μl, which increased to >200 cells/μl at the first follow up following initiation of HAART were defined to have IRIS.…”

“…HAART has been linked to increased occurrence of autoimmunity and autoimmune disorders [ 13 , 14 ]. IRIS has been defined as an increased CD4 count above 200 cells/mm 3 in patients who previously had CD4 counts lower than 100–200 cells/mm 3 [ 15 , 16 ]. Hence patients in our study with baseline CD4 counts less than 200 cells/mm 3 , which increased to >200 cells/mm 3 at the first follow-up following initiation of HAART, were defined to have IRIS.…”
