How Caloric Vestibular Irritation Influences Migraine Attacks

Abstract: Stimulation of the vestibular system by cold irritation of the ear was performed in 12 patients during a migraine attack. In eleven of the subjects the headache was changed. The changes varied from completely disappeared to a slight decrease. The duration of the changes was either several minutes, during the vestibular irritation, or days. All changes were related to the induced sensation of vertigo. We discuss the probable mechanisms of these vestibular influences.

“…The main mechanism of VM is currently considered as the connection of trigeminal caudal nucleus with vestibular nucleus (Furman et al, 2013;Huang et al, 2020), so the vestibular stimulation and migraine headache could interact with each other. A study showed that headaches completely disappeared or reduced in strength after vestibular thermal stimulation during migraine attacks (Kolev, 1990), which suggested that the painful conduction pathway might be inhibited after the activation of the vestibular pathway. On the other hand, both the prevalence of migraine and VM was significantly higher in females than males, and Group A: the onset of migraine was earlier than vertigo; VAS: visual analog scale.…”

“…33 Many authors have applied vestibular and auditory testing in patients diagnosed with migraine-related auditory-vestibular dysfunction in an attempt to establish a pattern for vestibular findings that would support the diagnosis of this condition. Another aim of vestibular testing would be to differentiate migraine-related auditory-vestibular dysfunction from the initial stages of MS. 16,20,22,23,36,37,38 …”

“…Furthermore, recent transcranial Doppler sonography data demonstrate that CVS treatment with the device elicits changes in cerebrovascular dynamics that point to brainstem neuromodulation . Second, irrigation calorics have been shown, albeit in small, uncontrolled samples, to acutely mitigate pain, including pain experienced during migraine attacks …”
