How guideline development has informed clinical research for organic acidurias (et vice versa)

Abstract: Organic acidurias, such as glutaric aciduria type 1 (GA1), methylmalonic (MMA), and propionic aciduria (PA) are a prominent group of inherited metabolic diseases involving accumulation of eponymous metabolites causing endogenous intoxication. For all three conditions, guidelines for diagnosis and management have been developed and revised over the last years, resulting in three revisions for GA1 and one revision for MMA/PA. The process of clinical guideline development in rare metabolic disorders is challenged… Show more

“…Liver transplantation has emerged as an effective therapeutic approach for selected PA patients. However, recent disease management guidelines emphasize the importance of several other factors and advances in acute and chronic care when assessing survival outcomes [ 5 , 6 ]. Improved treatment strategies and patient monitoring, particularly focusing on metabolic stability, play a crucial role in determining long-term outcomes for transplanted PA patients as well and may continue to improve survival post-transplant.…”

“…The development of clinical guidelines can directly improve management, bring further insights, and advance research. Guidelines for the management of pregnancies in OADs are limited, consistent with the limited experience to date [107]. Clinicians should be aware of potential complications and carefully consider how best to manage these conditions during pregnancy.…”

“…Boy et al 10 present the third revision of recommendations for glutaric aciduria type 1, developed by an international group of experts and patient representatives. Finally, Forny et al 11 demonstrate how guideline development can guide clinical research (et vice versa), enabling a steady improvement of our knowledge base and the level of evidence of recommendations for these rare diseases.…”
