Abstract: Polycystic ovary syndrome (PCOS) is one of the major causes of decreased fertility in young females. Multiple extra- and intra-ovarian factors which are worked together or separately are responsible for sub-fertility. However, still, ovulatory dysfunction is the cornerstone that occurs as a result of hormonal dysfunction and consequently affects oocytes' quality, embryonic development and finally, implantation failure with a significant number of females seek assisted reproduction for getting pregnancy.

Objectives Women with PCOS often experience significant difficulties in achieving spontaneous pregnancy. Intracytoplasmic sperm injection (ICSI) is a viable treatment option for these patients, offering an acceptable success rate. This study purposes to identify factors that may positively or negatively influence pregnancy rates in PCOS women undergoing ICSI and explore potential modifications to enhance successful pregnancy outcomes. Methods The study included ninety sub-fertile couples with female partners with diagnosed PCOS. Comprehensive evaluations of the partners included medical history, physical examination, hormonal analysis, transvaginal ultrasound (TVUS), and seminal fluid analysis. All couples underwent ICSI. Pregnancy rates were determined by positive pregnancy tests 14 days after fresh embryo transfer, and participants were divided into two groups: pregnant and non-pregnant. Results Of the 90 women who underwent ICSI cycles, 24 achieved pregnancies, resulting in a pregnancy rate of 26.66 %. Non-pregnant women had significantly higher body mass indices (BMI). Additionally, women with elevated cycle day 2 serum estradiol (E2) levels and low follicle-stimulating hormone (FSH) levels exhibited significantly lower pregnancy rates. Women whose male partners had abnormal semen parameters also demonstrated significantly lower pregnancy rates. Conclusions Several factors negatively impact pregnancy rates in PCOS women undergoing ICSI, including high BMI, elevated E2, low FSH levels on cycle day 2, a lower number of mature oocytes, and male factor sub-fertility. Many of these factors can be mitigated through the use of ICSI, thereby improving the chances of achieving a successful pregnancy.

Polycystic Ovary Syndrome is a chronic endocrine disorder with clinical manifestations of oligomenorrhoea, amenorrhea, hirsuitism, ovarian dysfunction, and multiple ovarian cysts, affecting many women of reproductive age. PCOS is the primary cause of ovulatory infertility worldwide. This study assessed the concentration of reproductive hormones in polycystic ovarian syndrome patients attending tertiary hospitals in the Niger Delta Region, Nigeria. Three hundred and fifty infertile premenopausal women aged 20 to 40 years were recruited; comprising 250 women presenting complete Rotterdam patients of polycystic ovarian syndrome diagnostic criteria, and 100 apparently healthy women who serve as the control. Blood samples were collected and analyzed for reproductive hormone levels using an enzyme-linked immunosorbent Assay. The result showed significantly increased concentration of estradiol, luteinizing hormone, LH/FSH ratio, testosterone, and prolactin levels in the PCOS patients when compared with the control subjects. Progesterone and Follicle-stimulating hormone levels were significantly lower in the patients with PCOS than in normal subjects. However, there was no significant difference was observed in the serum dehydroepiandrosterone levels. Furthermore, the study revealed that insulin resistance level was significantly (p=0.000) higher in the PCOS patients than in the control subjects. The BMI was significantly higher in PCOS patients than in the control group. The study result showed a positive correlation between BMI and insulin resistance, and a negative correlation with FSH. The result showed a significant positive correlation between estrogen and insulin resistance, Luteinizing hormone, and a negative correlation with LH/FSH ratio. The study affirms that there is an interrelationship between hormones thus, promoting hormonal disorders in polycystic ovarian syndrome patients.
