How well do questionnaires perform compared with physical examination in detecting flexural eczema? Findings from the International Study of Asthma and Allergies in Childhood (ISAAC) Phase Two

Abstract: ISAAC questionnaire-derived symptom prevalences are sufficiently precise for comparisons between populations. Where diagnostic precision at the individual level is important, questionnaires should be validated and potentially modified in those populations beforehand, or a standardized skin examination protocol should be used.

“…Nevertheless, the ISAAC questionnaire used in most of the self-reported studies has been validated and used widely in epidemiologic studies regarding allergy in children. [87][88][89] In conclusion, overweight/obesity is associated with an increased prevalence of AD, particularly in children and adults in North America and Asia. Future studies are needed to address the knowledge gaps about the association of overweight/obesity and AD, including studies using more rigorous clinical criteria for AD, such as the Hanifin and Rajka criteria, 28 longitudinal studies to determine the temporal relationship, and translational studies to explore the mechanisms of association.…”

“…Diagnosis of eczema via a questionnaire can be advantageous, especially in epidemiological studies that require large sample sizes, but also in clinical practice [9,10]. Validated eczema questionnaires have been available for schoolchildren [11,12], but the incidence of eczema is highest in pre-school children, with a peak during infancy.…”

“…However, self-report of eczema using different instruments has been validated and found to have good correlation with results of a clinical examination. 37,38 , and an interaction term between them. Multivariate models included history of or current asthma, history of or current hay fever, age, race/ethnicity, household income, highest level of educational attainment in the household, US birthplace, number of persons in the home, and use of prescription medication.…”
