Background: MMP-9 plays a significant role in invasion and migration of tumor cells and metastasis. A combination MMP-9 biomarker with HER2 and Ki-67 is expected to provide more specificity in prognosis for better breast cancer (BC) treatment options. Methods: A retrospective cohort of 34 patients with HER2 enriched breast cancer were studied from January 2016 until December 2020. Assessment MMP-9 by IHC using Monoclonal Mouse Anti-Human MMP-9 Antigen with semiquantitative immunoreactive scores methods was done. Results: The samples included patients aged 29 to 66 years. Patients’ educational level was mostly high school graduation (n=17; 50%). Distant metastases occurred in 10 (29.4%) patients, histopathological Grade II was found in 29 (85.7%) patients, positive LVI was found in 18 (52.9%) patients, and high proliferation rate (Ki67 > 20%) was found in 32 (94.1%) patients. All the patients underwent MRM with a history of chemotherapy in 29 (85.3%) patients, radiotherapy in 14 (41.2%) patients, and targeted therapy in only seven (20.6%) patients. Most of the patients had locally advanced stage III (n=21; 61.8%). The MMP-9 High expression was found in 20 (58.8%) patients and 14 (41.2%) patients had a low expression. A significant relationship was found between MMP-9 expression and DFS (p=0.023); while a significant relationship was found with OS (p=0.093). The mean DFS for High expression MMP-9 was 37.3 months and 45.3 months for low expression. The mean OS was 37.6 months for High-intensity MMP-9 and 42.7 months for Low-intensity. Conclusions: The MMP-9 expression (p=0.037), age group<40 years old (p=0.024), and the history of radiotherapy (p=0.035) were significantly related to he occurrence of distant metastases. The MMP-9 High expression had a 7.5 times greater risk of distant metastases. Impacts: The MMP-9 can be used as a prognostic factor for distant metastasis in HER2 Enriched BC.

Background : Immunotherapy is widely used to increase the response rate of chemotherapy drugs. Eleuhterine palmifolia (L) Merr or dayak onion are known to contain phytochemical compounds that have antiproliferative effects, and have been used as anti-cancer empirical therapies. This study aimed to prove the antiproliferative effect of Eleuhterine palmifolia (L) Merr extract in Sprague-Dawley rats with breast cancer. Methods : This study used 18 Sprague-dawley rats, divided into 3 groups, K, P1 and P2. K was given regular feed, P1 received adriamycin 5 mg/kgBW single dose, P2 received adriamycin 5 mg/kgBW single dose and extract of dayak onion 105 mg/kgBW/day for 3 weeks. Results : Difference in tumor size before and after treatment was 7.2 ± 2.57 mm (K), 3.23 ± 3.03 mm (P1), 3.14 ± 2.65 mm (P2), with p value is 0,157 (p>0,05). Ki-67 index calculations are 11.1 ± 7.27% (K), 9.64 ± 6.99% (P1), 9.58 ± 3.52% (P2), with p value is 0,704 (p>0,05). Conclusion : The addition of Eleuhterine palmifolia (L) Merr (dayak onion) extract did not cause a significant difference in tumor cell proliferation in Sprague-Dawley rats with breast cancer.

Background: Breast epithelial duct cells show response to sex hormone such as estrogen and progesterone since they have receptor for estrogen and progesterone. Consequently, malignant cells from breast epithelial ductuli (Breast Ductal Carcinoma) show positivity for estrogen and progesterone receptor (ER and PR). However, when this neoplasm becomes poorly differentiated, they lost these characteristics and became ER and PR negative. On the other hand, poorly differentiated cancer cells show more nuclear enlargement that was accompanied by increase nuclear pleomorphism. This study wants to compare area of breast cancer cells nuclei and nuclear pleiomorphism in positive and negative ER and PR. Material and method: Positive and negative ER and PR breast cancer cases were collected from archive of Anatomic Pathology Department each consisting of ten to fourteen cases. Image analysis was made from each case to measure the area, diameter, and nuclear pleiomorphism. Result and discussion: There were no differences in nuclear size and pleomorphism of breast cancer cells with positive and negative ER and PR. This result show that poorly differentiated cancer cells did not accompany by increase of nuclear anaplasia. Conclusion: There is no difference of cancer cells morphology in positive and negative ER and PR. Hormonal status and anaplasia of cell nuclei can be used as independent prognostic factor to breast cancer progression.
