Human NOTES Cholecystectomy: Transgastric Hybrid Technique

Abstract: NOTES hybrid transgastric cholecystectomy can be performed safely in human patients. This procedure is still technically challenging given the current instrumentation that is available. In order to perform a pure NOTES transgastric cholecystectomy, a safe blind access method, improved retraction, endoscopic hemostatic clips, and reliable closure methods need to be developed.

“…Until 31 August 2009 many NOTES procedures on a porcine model or in humans were performed, for example: cholecystectomy, appendectomy, staging procedures, hemicolectomy, adhesiolysis, gynaecological or bariatric procedures [2,[4][5][6][7][8][9]. There have been 28 pure NOTES and 183 hybrid NOTES (laparoscopically assisted NOTES) procedures published so far.…”

“…Early experiences with transgastric NOTES have included diagnostic peritoneoscopy and organ biopsy [28,29]. Cholecystectomy has been the most commonly performed transgastric procedure, whereas it has been exclusively performed by hybrid means [30,31,32,33,34,35]. Due to anatomical restrictions, gallbladder retraction may only be accomplished by laparoscopic instrumentation in order to achieve the critical view, although details on endoscopic retraction have only been sparsely and insufficiently described in the literature [30,31].…”

“…In 2004, the first human NOTES operation was reported when an appendix was removed through the mouth. In the United States, currently reported studies have included the use of diagnostic peritoneoscopy [17] and transvaginal [18] and transgastric cholecystectomy [19] . Internationally, use of NOTES in humans continues in countries such as India, Japan, Turkey, Japan, South America, and France.…”
