Hybrid techniques and patients’ safety in implementing transoral sublingual thyroidectomy

Abstract: The transoral access seems to be less invasive than other extracollar endoscopic accesses in thyroid surgery. For a sublingual single-access routine surgery, better instruments are needed. The vestibular access is possible with standard instruments. Using hybrid technologies for implementation should not be considered as a failure. The transoral thyroidectomy can be a safe method in the hand of experienced surgeons. We expect this hybrid technique to play a major part in further spreading endoscopic transoral … Show more

“…Combining the transoral technique with other minimally invasive techniques has also been proposed. 41 Although the latter procedures may be easier, they have not been included in this systematic review, because they go against the scarless principle of the transoral technique.…”

“…The most important advantages of remote access robotic or endoscopic thyroidectomy over conventional transcervical thyroidectomy include superior aesthetic results and the improved surgical views obtained with magnification. [9][10][11][12][13][14][15][16] Of these, the oral vestibular approach has been most popular recently. [5][6][7][8] Recently, transoral thyroidectomy has been increasingly adopted by several institutions.…”

“…Transoral thyroidectomy includes the sublingual approach and the oral vestibular approach with or without CO 2 insufflation. [9][10][11][12][13][14][15][16] Of these, the oral vestibular approach has been most popular recently. However, the efficacy and feasibility of transoral robotic and endoscopic thyroidectomy has not been fully assessed although some institutes have reported its feasibility and safety, [17][18][19][20][21][22][23][24] and one study obtained comparable surgical outcomes to conventional transcervical thyroidectomy.…”

“…Transoral thyroidectomy is a form of NOTES (natural orifice transluminal endoscopic surgery) [ 16 ] technique with minimal invasiveness and no scar on body surface (Fig. 7 ), including vestibular approach and sublingual access [ 17 , 18 ], but the latter is obsolete because of the structural damage it may cause. The thyroid gland is a solid organ rich in blood, is adjacent to the complex anatomical structure and important blood vessels and nerves, and has no natural space around it.…”
