Hydatid Cyst of Spleen Presenting with Vague Symptoms: A Diagnostic Conundrum

Abstract: Hydatid cysts are caused by a tapeworm Echinococcus granulosus. They usually occur in the liver. When occurring in spleen they present with vague symptoms which make it difficult for the physicians to diagnose. We present a case of a 10-year-old male who presented with fever, abdominal pain, and burning micturition along with vomiting. Abdominal exam revealed no visceromegaly. Abdominal ultrasound (US) and computed tomography (CT) scan showed multiple hypoechoic and hypo-dense areas, respectively. Splenic absc… Show more

“…At times it can present with vague symptoms of fever, burning micturition and vomiting with no splenomegaly. 5 Hydatid cyst is common in liver and lungs and can affect other organs like spleen concominant with the lung and liver as in our case but isolated cases of splenic hydatid cyst are also recorded. 6 The imaging and histopathological findings of splenic hydatid cyst are similar to those of hydatid cyst of liver or lungs.…”

“…La serología contribuye solo en determinados casos, por presentar un porcentaje elevado de falsos negativos (5) , siendo de mayor utilidad la ultrasonografía abdominal por su elevada sensibilidad (90-95%) (9) . Asimismo, la tomografía abdominal puede determinar la localización y medidas del quiste, características, contactos anatómicos y mostrar otras posibles lesiones (10) . Sin embargo, está contraindicado realizarla durante la gestación por el riesgo de irradiación al feto.…”

“…It can directly identify the cyst, specify its size, location and detailed anatomic contacts with surrounding vessels and organs, and it can look for other concomitant intra-abdominal localizations and evaluate possible complications [ 10 , 12 , 13 ]. The diagnosis is strongly guided by some suggestive signs such as parietal calcifications, floating membrane, daughter cysts and the presence of other concomitant extra-splenic cystic images [ 11 , 14 ]. The CT is also a more powerful imaging tool used to detect recurrences [ 10 , 13 ].…”
