Hydatid cyst of the pancreas: a case report with 5 years' follow-up

Abstract: Primary hydatid disease of the pancreas is rare. The authors report a 3-year-old girl who had a cystic mass in the head of the pancreas with no other viscera involved. At laparatomy the entire endocyst was totally extracted from the surrounding pancreatic parenchyma. The postoperative course was uneventful. She is currently disease-free with a follow-up of 5 years. Only four other pediatric cases have been reported in the English literature.

“…In pancreatic disease, it's essential to maintain the organ's exocrine and endocrine functions. Many surgical techniques are available to remove the cyst; however pericystectomy with drainage of the residual cavity is the technique of choice [4,8]. Because of the localization of the cyst near a major biliary or vascular structures and high risk of pancreatic fistula [9], total pericystectomy cannot be done in some cases.…”

“…In our case, size of cyst was large hence we suspected communication with pancreatic duct and did internal drainage as this appeared to be the safest option. Distal pancreatectomy with splenic conservation is the treatment of choice for hydatid cyst localized in the tail of pancreas [4]. Percutaneous treatment of pancreatic hydatid cyst using catheter technique has also been reported in few properly selected cases [10].…”

“…3,4,7 In cases with risk of pancreatic fistula, partial pericystectomy or insertion of a stent into duct during surgery is recommended. 3,4 Distal pancreatectomy with the splenic conservation is the recommended for hydatid cysts localized to the tail of the pancreas.…”

“…Although hydatid cysts are mostly found in the liver and lung, it can arise anywhere in the body. Primary hydatid cyst of the pancreas is extremely rare, especially in childhood (1)(2)(3)(4). The preoperative diagnosis is very difficult.…”
