Hydrosurgical debridement versus conventional surgical debridement for acute partial-thickness burns

Abstract: This is a protocol for a Cochrane Review (Intervention). The objectives are as follows:To assess the effects of hydrosurgical debridement and skin grafting versus conventional surgical debridement and skin grafting for the treatment of acute partial-thickness burns.

“…Hydrosurgical debridement is a novel modality, which enables the surgeon to precisely cut and remove nonviable tissue without causing collateral damage to healthy surrounding tissue using highly pressurized saline solution emitted from the tip of a hand-held instrument [25] . The jet travels through the nozzle of the instrument generating a Venturi effect which is an increase in the fluid's velocity and a decrease in its static pressure due to its passage through a constricted area [26] . Here, in this case, we have used jet force technology (JFT), which is a type of hydrotherapy previously studied extensively in our department and shown to be an effective means to debride the wounds without the mess of traditional methods and hasten wound healing [27] .…”

“…Only five Cochrane Database Systematic Reviews examining wound debridement and the effectiveness of the modality were published between 2010 and 2020 (Supplemental Table 1). [17][18][19][20][21] Note, however, that the Cochrane reviews require a very high level of evidence, and there are many more systemic and scoping review articles that mention debridement.…”

“…The surgical management of burns aims to facilitate timely wound healing, ideally within 2 weeks, to maximize function of the affected area and minimize pathological scarring. Once the burned skin has been debrided (using a blade, hydrosurgery or enzymatic debridement) there may be insufficient healthy dermis remaining to achieve spontaneous wound healing within an appropriate timescale. In this situation, an autologous split‐skin graft (SSG) may be used to reconstruct the wound bed.…”
