Hyoid osteoradionecrosis associated with carotid rupture: Report of 2 cases

Abstract: ORN of the hyoid can be associated with life-threatening carotid rupture. In patients with a history of head and neck irradiation who present with painful swelling in the region of the hyoid, this entity should be considered. Early intervention may prevent subsequent rupture of the carotid artery and hemorrhage.

“…Hyoid bone ORN is a rare entity described in only 11 patients within the otolaryngology literature (Table 1) [10][11][12][13][14][15][16][17] . All cases had received RT doses ranging from 60 to 80 Gy for various stages and sites of primary HNC.…”

“…The importance of early recognition of hyoid bone involvement was highlighted by Chard et al reporting two cases of HB-ORN in association with carotid pseudoaneurysm and rupture requiring surgical intervention 13 . Treatment modalities reported amongst the 11 cases ranged from conservative management including observation and hyperbaric oxygen to surgical resection and reconstruction.…”

“…Osteoradionecrosis (ORN) of the hyoid bone is an infrequent and less critical complication, as compared with that of the jaw and thyroid cartilages, associated with chemoradiation treatment for oropharyngeal and laryngeal cancer [ 1 , 2 ]. However, ORN of the hyoid bone sometimes induces severe front neck infection, laryngeal stenosis and carotid rapture [ 3 , 4 ]. Most reports of ORN of the hyoid bone involve small clinical cases, and there has been only one report of a series of CT image studies [ 5 ].…”
