Hypereosinophilia as a paraneoplastic phenomenon in non-small cell lung carcinoma

Abstract:
Hypereosinophilia is a rare paraneoplastic nding in malignant disease, particularly lung cancer. When it occurs, it is usually indicative of
metastatic disease. We describe a 52-year-old male patient with paraneoplastic hypereosinophilia associated with primary adenocarcinoma
of the right lower lobe and extensive metastatic disease.

“…Sometimes a combination of steroids and hydroxyurea can increase the response rate [ 6 ]. However, the cornerstone of management includes treating the malignancy by either chemotherapy or surgery [ 4 , 9 ]. It is worth mentioning that the presence of paraneoplastic eosinophilia is associated with a poor prognosis and a more aggressive tumor [ 4 ].…”

“…Eosinophilia is defined as an absolute eosinophil count (AEC) of more than 500 cells/µL. Hypereosinophilia is a more severe condition and is defined as having an AEC greater than 1500 cells/µL, without organ damage [ 2 - 4 ]. There are many causes of eosinophilia which include parasitic infections, allergic diseases, rheumatologic diseases, and neoplasms.…”

“…Among the one hundred previously reported and analyzed by us cases of TABE, the death of 68 patients was reported ( 4 , 7 , 8 , 10 – 14 , 16 – 22 , 24 – 28 , 30 , 32 , 34 37 , 39 , 44 , 46 – 51 , 53 60 , 65 , 67 69 , 71 , 72 , 74 76 , 82 – 88 , 94 96 ). About 90% of the deaths occurred within one year after the diagnosis of paraneoplastic hypereosinophilia.…”

“…It has been associated with a better prognosis in certain types of cancers like colorectal and esophageal squamous cell carcinoma, but with a poor outcome in patients with Hodgkin's lymphoma. 1 Concerning lung cancer, 3,4 and specifically nonsmall cell lung cancer (NSCLC), with the most common sub-type being lung adenocarcinoma, 2 hyper-eosinophilia has been consistently associated with extensive metastasis, poor response to therapy and overall a worse prognosis, particularly when a paraneoplastic process is functioning as the cause. 3 Pembrolizumab, one of the immune checkpoint inhibitors (ICI) used in the treatment of lung cancer, have recently revolutionized treatment in oncology, showing evidence of improved survival compared to conventional platinum-based chemotherapy in lung malignancy.…”
