Hypertension, dyslipidaemia, and cardiovascular risk in HIV-infected adults in Poland

Abstract: Incidence of hypertension in particular age groups of HIV infected people is higher than in the general Polish population. Hypertension is influenced by traditional risk factors and duration of HIV infection but not antiretroviral treatment. HIV/HCV coinfection appears to be protective against hypercholesterolaemia.

“…In our study, the median BMI was high, indicating that participants had excess body weight. Being overweight is a modifiable risk factor that was found to be associated with hypertension in the current study, which corroborates with the findings of a study conducted in Poland and another in Nigeria 17,18 . Smoking, another modifiable CVD risk factor, was found to be significantly associated with the hypertensive status in our study population.…”

“…However, HCV is known to disturb the glucose metabolism by direct and indirect influence on intracellular insulin signalling, which leads to insulin resistance, impaired glucose tolerance, or even type 2 diabetes mellitus [23]. Similarly to Cheng et al [24], we also did not observe any difference in MS incidence between populations with and without HCV coinfection, despite the known direct cytopathic influence of HCV on lipid profile resulting in reduction of cholesterol and/or TG levels [25]. These perturbations seem to resolve after successful viral clearance.…”

“…27 Again, a recent case-control study from hypertension are not immediately apparent to us. We consider this finding unexpected given that hypertriglyceridemia was significantly associated with hypertension in a Polish HIV cohort 40 and in the general population. 41 In the Polish HIV cohort, 40 the authors noted though that hypertriglyceridemia was highly associated with protease inhibitor (PI)-based cART, but in this study, >98% of cART-exposed PLWH were on non-nucleoside reverse transcriptase inhibitor (NNRTI)-based cART.…”
