Hypertension trials update

Abstract: Hypertension is one of the most prevalent cardiovascular diseases and its treatment requires multimodal therapeutic approaches. This review aims to provide a summary and update on relevant evidence in hypertension research published in 2019/2020. These include trials dealing with the prognostic effect of systolic and diastolic blood pressure values, the association between hypertension and valve disease, reproducibility of masked and white-coat hypertension, and the prognostic importance of ambulatory and nigh… Show more

“…Importance of nighttime blood pressure 24-h ABPM plays an incremental role in the management of hypertension as it provides information on circadian variations in BP and can identify white-coat and masked hypertension [6]. A reduction in SBP by 10e20% during nighttime is considered physiological dipping.…”

“…In the 2017 American College of Cardiology and American Heart Association Guidelines, hypertension has been redefined as a systolic blood pressure of ≥130 mmHg and/or a diastolic blood pressure of ≥80 mm [ 5 ]. The shift of hypertension definition led to an increase in hypertension prevalence from 18.9 to 43.5% [ 6 ], which may augment the reported HHD burden.…”

“…Dudenbostel et al reported that 90% of male patients and 77% of female patients with resistant hypertension had obstructive sleep apnea, while nocturnal hypertension is significantly higher in patients with OSA and is associated with the risk of total cardiovascular disease, cardiovascular death, heart failure, stroke and myocardial infarction ( Dudenbostel and Calhoun, 2012 ). A 20 mmHg increase in night-time systolic blood pressure was significantly associated with a 36% increase in cardiovascular event risk and a 23% increase in mortality ( Al Ghorani et al, 2021 ). Effective control of co-morbidities, optimal antihypertensive drug treatment, continuous positive airway pressure therapy, and other therapeutic modalities are substantial in improving hypertension control and reducing cardiovascular risk.…”
