Abstract:Hypertrophic osteoarthropathy is characterized by the combination of clubbing, arthropathy and periostitis. This paraneoplasia may be the presenting manifestation of occult malignancy. The diagnosis of hypertrophic osteoarthropathy should cause an intensive search of cancer. Further associated diseases are discussed.
“…These observations suggest that arthritis might be an early manifestation of HOA and usually precede radiographic signs of periostitis. For patients with HOA, lung cancer was the most common underlying neoplasia (5 of 6; 83.3%) and this percentage is similar to those reported by other groups 16 17. Paraneoplastic arthritis is usually described as refractory to NSAIDs and steroids.…”
Underlying neoplasia should be considered in male patients with new onset polyarthritis, smokers, and particularly in patients chronically ill. Additional investigations should then be performed to diagnose cancer at an early stage.
