Hypocalcemic Laryngospasm and Tetany in a Child With Renal Dysplasia

Abstract: Stridor is a common presenting sign of respiratory illness in the pediatric population-especially in the winter. Infrequently, tetany as a chief complaint may be seen with tetanus, seizures, and dystonic reactions to medications. There are few medical conditions that present with both symptoms. This is a case of a patient who presented to our emergency department in early winter with both stridor and tetany. The child was diagnosed with hypocalcemia resulting from a previously undiagnosed renal dysplasia and h… Show more

“…Laryngospasm is a common presentation of hypocalcaemia in infants,68 but is rare in adults and mostly seen in patients with pseudohypoparathyroidism caused by hypomagnesaemia 810. In our patient, the hallmark of hypocalcaemia, tetany, was not present nor were the clinical tests positive for latent tetany despite severe hypocalcaemia.…”

“…In our literature search we could find only one case report of a child presenting with hypocalcaemic laryngospasm secondary to renal dysplasia 7. Nephronophthisis is an autosomal recessive form of cystic renal disease, which can present as chronic renal failure in infancy, childhood or adolescence.…”

“…[2] Hypocalcemic laryngospasm causing stridor and tetany in a child with renal dysplasia has also been described. [3] A fatal case of biphasic wheezing in an infant attributed to hypocalcemia has been described in literature. [4] Vitamin D by itself has been shown to have beneficial effects in children who wheeze by up regulating antimicrobial proteins or through multiple immune effects.…”
