Hypogammaglobulinaemia secondary to cow‐milk allergy in children under 2 years of age

Abstract: Summary Symptomatic hypogammaglobulinaemia in children younger than 2 years of age was studied to rule out a primary immunodeficiency. Thirty‐four patients were referred to the Immunology Service to study the hypogammaglobulinaemia‐associated clinical picture. Food allergy was documented in 10 patients by personal and familial history, presence of specific immunoglobulin E (IgE) and elevated total serum IgE levels. Coeliac disease and human immunodeficiency virus infection were also ruled out. Protein loss thr… Show more

“…Another symptom associated with IgE-mediated CMA is transient hypogammaglobulinaemia in infancy, which is characterized by reduced IgG and IgA antibody levels and preserved functional antibody response. 61 Children with primary immunodeficiencies such as hyper-IgE syndromes can also present with CMA in the context of these conditions. 62,63 …”

“…Except for the low levels of IgG and IgA, the other tests performed to evaluate PID and CMA were normal. Hypogammaglobulinemia secondary to CMA in children was described by Bezrodnik et al, 2007 [29]. In their patients faecal α -1-antitrypsin was elevated in 4/5 patients, leading to hypogammaglobulinemia, and in all five cases the patients had high levels of total IgE and S-IgE.…”

“…Food allergies have been reported in 5.9% of patients with transient hypogammaglobulinemia in infancy [ 25 ]. Food allergies, especially cow’s milk allergies, are common in infants and may coexist with antibody production defects [ 26 ]. Hypogammaglobulinemia may be related to the leakage of immunoglobulins through the inflamed mucosa, and normalization of immunoglobulin levels was observed with improvement of symptoms after exclusion of the allergens or treatment of colitis in two of our patients.…”
