We reported an Idiopathic Thrombocytopenic Purpura (ITP) patient, 66-years-old (woman), with hypokalemia. She received 125 mg three times daily of methylprednisolone injection for her ITP. Corticosteroids are the initial treatment of ITP. Her potassium level decrease after she took methylprednisolone. Hypokalemia is also a common problem affecting the elderly or geriatric population. Many literatures reported side effects of corticosteroid is associated with hypokalemia. Monitoring potassium levels must be check during corticosteroid therapy. In this report, we describe the association between corticosteroid with hypokalemia effect.

The article concerns one of the common adverse effects during treatment — steroid myopathy. The information about pathogenic specifics of myopathy development in administration of glucocorticoids, the most typical clinical manifestations are described, and results diagnostic methods with estimation of a role of enzyme level evaluation, electromyography, ultrasound study of the muscle tissue, computer and magnetic resonance tomography. There is description of muscle weakness development in 49-year old woman who has been receiving methylprednisolone 88 mg/day due to revealed thrombocytopenia. One week after the treatment was started the patient experienced onset and progression of muscle weakness limiting her motion and self-maintenance. After performing of investigation including electromyography steroid genesis of myopathy was suggested. The patient’s condition began to improve after disc ontinuation of glucocorticoids and administration of calcium supplements, vitamin D, and anabolics, and the patient was discharged.

Hypokalemia is one of the most frequent electrolytic disturbances encountered in clinical practice. It usually presents with symmetrical generalized muscle weakness and, on occasions, with arrhythmias. There are scarce reports of cases presenting with asymmetric or focal weakness in the literature and no systematic reviews on the subject. Therefore, our aim is to describe 2 cases of hypokalemic paralysis that presented as monoparesis and to review the literature on focal hypokalemic paralysis. Hypokalemic paralysis is usually reversible. However, it can be fatal if the diagnosis and treatment are delayed. It is important to take into account this presentation because failure to recognize it could lead to misdiagnosis, delaying the adequate treatment.
