Acanthamoebascleral abscess reoccurring after penetrating keratoplasty

Abstract: Two months following penetrating keratoplasty for Acanthamoeba keratitis, a 76-year-old man was referred due to inability to wean high-dose topical steroids. Despite a very healthy graft and minimal pain, a scleral abscess involving three clock hours of the superior conjunctiva was present. The patient underwent conjunctival and scleral excision of the area of apparent infection with adjuvant mitomycin C and double freeze–thaw cryotherapy treatment followed by amniotic membrane graft. Recurrence was confirmed … Show more

“…Medical management is generally attempted prior to surgery unless there is immediate threat to central nervous system penetration. Procedures such as penetrating keratoplasty (PKP) and deep anterior lamellar keratoplasty (DALK), which were originally performed with the intention of debriding diseased tissue and debulking disease, are now generally reserved for either cases refractory to medical management or for restoration of vision due to structural damage [6,19,20].…”

“…Surgical excisions were performed in our case for the following reasons. First, the response to the medication for infection control was poor, excision of the conjunctiva has been reported as successful for infection control,[ 15 , 16 ] and to explore the possibility of scleral involvement. In our case, the area involved had extended posteriorly into the orbit inferiorly.…”

“…3,4 The literature is relatively sparse regarding the management of AS. Barnett et al 5 reported a good visual outcome in a patient who developed AS after penetrating keratoplasty (PK) when using extensive sclerectomy, adjuvant mitomycin C, and amniotic membrane graft, followed by a prolonged course of oral and topical antiamoebic therapy. Similarly, the literature is also sparse concerning the prognosis of those patients suffering from AS.…”
