We experienced an extremely rare case in which combined antibacterial therapy for a non-tuberculous mycobacteria (NTM) infection of the parotid gland achieved a favourable outcome in an elderly immunocompetent patient. Although a 79-year-old man, who presented with swelling and fistula formation in the left parotid gland region, initially received combined antituberculous therapy due to a positive result of acid-fast staining, the lesion did not respond to these agents. Thereafter, since the culture examination did not detectMycobacterium tuberculosisor NTM, we excluded tuberculosis and considered the possibility of an NTM infection caused by a rare mycobacterial species. Therefore, we switched to the clarithromycin-based antibacterial treatment for eight consecutive months without a surgical intervention, resulting in the complete disappearance of the lesion and no evidence of recurrence detected for 4 years. This conservative chemotherapy might be a feasible alternative to a surgical intervention for treatment against NTM infections of the parotid gland.

Tuberculosis of the parotid gland is rare and requires high degree of clinical suspicion. Because of its low incidence and nonspecific presentation mimicking benign and malignant tumors of the gland, it is usually missed preoperatively. Diagnosis is usually made in parotidectomy specimens removed for a tumorlike clinical diagnosis. The diagnosis should be considered in endemic regions and in developed countries with a reemergence of tuberculosis because of immunodeficiency caused by human immunodeficiency virus infection to avoid undue extensive surgery. Our case report of an immunocompetent patient with parotid tuberculosis emphasizes problems encountered in preoperative diagnosis. It also supports the role of polymerase chain reaction as an additional diagnostic modality in paucibacillary cases encountered in immunocompetent individuals because of a profuse granulomatous reaction.(Infect Dis Clin Pract 2008;16:261Y263)

Introduction We report a rare case of isolated parotid abscess due to Mycobacterium fortuitum in an immunocompetent adult female, which to the best of our knowledge (on internet search) is the first case of its type. Methodology Diagnosis was based on MTBDR CM assay (Hain's) culture, followed by a positive TBAg MPT64 culture for MOTT. The patient was treated with abscess drainage and antibiotics with good results. A parotidectomy was not required in our patient. Conclusion Mycobacterium fortuitum parotid abscess is very rare. A knowledge of the pathogenicity of this organism and careful culture methods seem to be the key of accurate diagnosis. Treatment protocols are still subject to research.
