PIEZO1 Mutation May Determine Early Onset of Clinical Manifestation of Anemia of Myelodysplastic Syndromes

Abstract: BACKGROUND: Anemia is the most frequent cytopenia in myelodysplastic syndromes (MDS), neoplastic diseases characterized by ineffective hematopoiesis and bone marrow dysplasia. MDS usually affect elderly individuals (median age in Italy 74 years), and quite rarely concern younger patients. It is of increasing interest to verify whether "young", not therapy-related MDS patients carry germline (GM) predisposition, in order to optimize therapeutic and transplant choices. The best approach is to perform Whole Exome… Show more

“…Marrow examination of second one, who presented with refractory thrombocytopenia and hemolytic anemia with few stomatocytes, showed major dyserythropoietic features including megaloblastosis and Howell-Joly bodies, although the presence of a profound vitamin B12 deficiency at diagnosis might enhance the findings [49]. Recently, whole exome sequencing performed in patients younger than 60 diagnosed with myelodysplastic syndrome and symptomatic anemia showed rare heterozygous missense variants of PIEZO1, likely pathogenic as osmolarity curves were impaired by ektacytometry, although associated with different MDS-related somatic variants [50] ] in PIEZO1 mutated HX showed that erythroid regeneration was lower than expected in pure hemolytic anemia but closer to BMRI in patients presenting type II congenital dyserythropoietic anemia (CDA-II), suggesting a part of ineffective erythropoiesis.…”
