IAPT and the internet: the current and future role of therapist-guided internet interventions within routine care settings

Abstract: Compared with the traditional face-to-face format, therapist-guided internet interventions offer a different approach to supporting clients in learning skills to manage and overcome mental health difficulties. Such interventions are already in use within IAPT (Improving Access to Psychological Therapies) and other routine care settings, but given their potential to deliver treatment more efficiently and therefore increase availability and access to evidence-based interventions, their use is likely to increase … Show more

“…In the context of lockdown measures, social isolation and physical distancing, face-to-face therapy is not practical or safe, therefore it is important to consider alternative methods in delivering treatment. There is a body of evidence indicating that computerised CBT is effective in child, young people and adult populations (Grist et al, 2018;Hollis et al, 2017;Newby et al, 2020;Olthuis et al, 2016;Thew, 2020;Vigerland et al, 2016). Evidence also suggests that transdiagnostic online CBT for anxiety in children and young people can be as effective in emergency contexts: in 2012, after a series of violent earthquakes in New Zealand, an online therapist-assisted CBT package was found to be effective at reducing anxiety (Moor et al, 2019).…”

“…For example, in the case of PTSD, they have recommended that there is enough evidence that online and videoconferencing trauma-focused CBT can be as effective as face-to-face therapy (NICE, 2018). Telemental health has been a topic of interest in IAPT services for several years (Thew, 2020). Furthermore, Whaibeh et al (2020) argue that telemental health has demonstrable effectiveness, convenience and reliability, and that it is an essential mode of service provision to be integrated into routine practice.…”

“…This format of therapy delivery has encountered scrutiny for its questioned likeness to 'real therapy'. More specific concerns have been raised about online facilitated therapy; for example, 'the therapeutic alliance won't be as strong online' or 'internet-based therapies won't be as effective as face-to-face therapy' (Thew, 2020;p. 4).…”

“…However, by and large these concerns have been counterbalanced by evidence that suggests therapy of this format is effective, albeit different from therapy as 'normal'. Above all, an undeniable strength of remote therapy, given social distancing, is its ability to enhance the availability and accessibility of psychological support (Thew, 2020). Similarly, telephone-delivered therapy has faced similar critiques, although research indicates that it remains able to provide the basis for a working alliance: a foundation of effective psychological support (Turner et al, 2018).…”
