Iatrogenic hepatic subcapsular biloma following PCNL: Diagnosis and management

Abstract: After the first description in 1976, percutaneous nephrolithotomy (PCNL) remains the treatment of choice for large renal stones. Adjacent organ injuries are rare complications, with pulmonary and colonic representing the most common, while hepatic represents one of the rarest. We describe our case of hepatic subcapsular biloma following a subcostal PCNL, conservatively managed by percutaneous drainage and endoscopic biliary stenting.

“…In this study, three cases of liver injury were examined, all of which underwent right PCNL and subsequently developed liver injury [22]. Hepatic complications are rare and often not given much attention during Am J Clin Exp Urol 2024;12(1):46-51 surgery [23]. This study emphasizes considering uncommon and rare complications and being prepared for their occurrence.…”

“…The second report involved a patient with a 2.2-cm stone in the lower pole of the right kidney. 2 On POD1 from PCNL, the patient developed right shoulder pain with erythema noted around the nephro-ureteral catheter. CT imaging demonstrated a small peri-hepatic free fluid collection.…”

“…Although percutaneous nephrolithotomy (PCNL) remains gold standard treatment for large renal stones, there exists minimal reports describing management of liver injury during PCNL. [1][2][3] Here, we present a multi-institutional series of cases of liver injury during PCNL.…”
