Ictal central apnea and bradycardia in temporal lobe epilepsy complicated by obstructive sleep apnea syndrome

Abstract: We describe the case of a 12-year-old boy who developed temporal lobe epilepsy (TLE) with daily complex partial seizures (CPS) and monthly generalized seizures. Moreover, he frequently snored while asleep since early childhood. Polysomnography (PSG) revealed severe obstructive sleep apnea with apnea–hypopnea index (AHI) of 37.8/h. Video-PSG with simultaneous electroencephalography (EEG) recording captured two ictal apneic episodes during sleep, without any motor manifestations. The onset of rhythmic theta acti… Show more

“…ICA occurs exclusively in focal epilepsy, emerging during 33–50% of focal seizures (Lacuey et al, 2018 ; Vilella et al, 2019 ; Tio et al, 2020 ). ICA can precede electrographic seizure activity as well as clinical seizure onset by up to 7–10 s (Nishimura et al, 2015 ; Tio et al, 2020 ). These apneas tend to be brief and do not substantially impact O 2 saturation (Bateman et al, 2008 ).…”

“…Так, описаны случаи эпилептических приступов, со-провождающихся развитием выраженной брадикар-дии, возникающих после эпизодов апноэ во время сна [21]. В одном исследовании показана высокая частота дыхательных нарушений, ассоциированных с эпизо-дами гипоксемии и брадикардии, у взрослых паци-ентов с верифицированной эпилепсией [22], что ука-зывает на необходимость проведения комплексного диагностического обследования для выявления НДС у этой категории больных.…”
