Identification of CTX-M15-, SHV-28-producing Klebsiella pneumoniae ST15 as an epidemic clone in the Copenhagen area using a semi-automated Rep-PCR typing assay

Abstract: Rapid molecular typing methods can be a valuable aid in the investigation of suspected outbreaks. We used a semi-automated repetitive sequence-based polymerase chain reaction (Rep-PCR) typing assay and pulsed field gel electrophoresis (PFGE) to investigate the relationship between local Klebsiella pneumoniae (K. pneumoniae) producing extended spectrum β-lactamases (ESBLs) and their relation to recognized Danish outbreak strains. PFGE and Rep-PCR produced similar clustering among isolates. Individual isolates f… Show more

“…ST15 together with ST326 caused the CG15 predominance among the rectal colonizers in GI in Barcelona (48.0%), in contrast to ESBL-or AmpC-producing K. pneumoniae clinical and carrier isolates from other Spanish centers (13,31). CG15, usually ST15 and/or ST14 (not found here), has been commonly observed with ESBLs or other ␤-lactamases worldwide (12,16,18,(32)(33)(34). CC147 was identified in four hospitals and was the largest CG/CC overall (n ϭ 42; 18.3%).…”

“…Beside several minor clones, three internationally disseminated CTX-M-15 producing K. pneumoniae clones, namely ST15, ST101 and ST147 were present in the Clinical Centre University of Pécs, of which ST15 and ST147 are considered to be epidemic clones in Hungary [6,[32][33][34][35][36].…”

“….the key challenge will not be to produce the sequence data, but to rapidly compute and interpret the relevant information from large data sets" (94). Easy-to-use criteria to define which isolates are within the same clonal lineage or within the same hospital outbreak are currently available for band-based techniques (23,40,48) but not for WGS (104). For this reason, we identified a threshold of 2,500 core SNPs that distinguished isolates of the same clonal lineage from those of different clonal lineages.…”

“…The Dice statistic was used, and band tolerance was set to 1%. Isolates clustering together at Ͼ90% similarity based on the UPGMA dendrogram were considered to be the same type (23,48,49). Whole-genome sequencing and assembly.…”
