Identifying Co-Exposure to Opiates and Gabapentin During Pregnancy

Abstract: Neonatal withdrawal can be difficult to treat in infants with co-exposure to opiates and gabapentin. Because maternal self-report can underestimate exposures, we evaluated the effect of universal toxicology screening for gabapentin. Identification of co-exposure to opiates and gabapentin increased after implementation of toxicology screening, with implications for improved neonatal care.

“…Previous studies on the use of antiepileptics during pregnancy have concluded that valproic acid, lamotrigine, and carbamazepine are associated with smaller head circumference observed in the fetus ( Margulis et al, 2019 ). Moreover, another study showed that gabapentin administration during pregnancy is associated with adverse gestational outcomes ( Loudin et al, 2020 ). Therefore, the safety of the usage of gabapentin during pregnancy has not yet been conclusively determined ( Loudin et al, 2020 ).…”

Therefore, the safety of the usage of gabapentin during pregnancy has not yet been conclusively determined ( Loudin et al, 2020 ). Thus, antiepileptics, such as gabapentin are generally prescribed according to the risk-to-benefit ratio ( Guttuso et al, 2014 ; Patorno et al, 2020 ).…”

“…Women with OUD experience other mental health issues and the need for psychotropic medications ( 13 , 14 ). Infants exposed to maternal opioids were more likely to require pharmacotherapy when co-exposed to benzodiazepines ( 15 ), tobacco ( 16 ), selective serotonin reuptake inhibitors ( 16 18 ), gabapentin ( 19 ), marijuana ( 20 ), or cocaine ( 21 ). The use of psychotropic medications in addition to prescription opioids increased the severity of NAS by two-fold compared to the use of prescription opioids alone ( 22 ).…”
