Identifying facilitators and barriers to develop implementation strategy for an ED to Ward handover tool using behaviour change theory (EDWHAT)

Abstract: Background Effective clinical handover is fundamental to clinical practice and recognised as a global quality and safety priority. Problems with clinical handover from the emergency department (ED) to inpatient ward across four hospitals in the Illawarra Shoalhaven Local Health District (ISLHD) were identified in a number of reportable clinical incidents. To address this, an ED to inpatient ward electronic clinical handover tool was developed and implemented. However, site uptake of the tool va… Show more

“…We also deemed that there would be a range (60–69%) which indicated it was neither a barrier nor an enabler. Seventy percent has previously been used as a cut point for being an enabler or barrier in a questionnaire informed by the TDF (Curtis et al, 2020). Descriptive statistics are presented.…”

“…Once considered the primary concern in healthcare (Clapper & Ching, 2020), communication now recognizes itself as just one of many significant challenges. Recent research emphasizes the importance of accountability (Curtis et al, 2020), conflict management (Fernández Castillo, Khalid & Salas, 2024); Keller et al, 2020), decision-making (Papermaster & Champion, 2021), and fostering progress through reflection and coaching (Zajac et al, 2021) within healthcare teams. These findings suggest that while communication remains crucial, it's imperative to take a holistic approach to competencies within healthcare teams, including handing over practices.…”

“…The handover process could be in verbal, written, or voice-recorded forms [ 3 ]. Several research studies have indicated that the handover process is usually associated with several problems, mainly high variability in the type of information that could lead to inefficiencies in clinical practice and suboptimal patient care [ 4 ]. Inappropriate handovers have many consequences, including, but not limited to, disruption of care, compromised patients' safety in the form of inappropriate drug dosing, surgeries on the wrong side, or even life-threatening side effects [ 5 ].…”
