PurposeNecroptosis is a mode of programmed cell death that overcomes apoptotic resistance. We aimed to construct a steady necroptosis-related signature and identify subtypes for prognostic and immunotherapy sensitivity prediction.MethodsNecroptosis-related prognostic lncRNAs were selected by co-expression analysis, and were used to construct a linear stepwise regression model via univariate and multivariate Cox regression, along with least absolute shrinkage and selection operator (LASSO). Quantitative reverse transcription polymerase chain reaction (RT-PCR) was used to measure the gene expression levels of lncRNAs included in the model. Based on the riskScore calculated, we separated patients into high- and low-risk groups. Afterwards, we performed CIBERSORT and the single-sample gene set enrichment analysis (ssGSEA) method to explore immune infiltration status. Furthermore, we investigated the relationships between the signature and immune landscape, genomic integrity, clinical characteristics, drug sensitivity, and immunotherapy efficacy.ResultsWe constructed a robust necroptosis-related 22-lncRNA model, serving as an independent prognostic factor for breast cancer (BRCA). The low-risk group seemed to be the immune-activated type. Meanwhile, it showed that the higher the tumor mutation burden (TMB), the higher the riskScore. PD-L1-CTLA4 combined immunotherapy seemed to be a promising treatment strategy. Lastly, patients were assigned to 4 clusters to better discern the heterogeneity among patients.ConclusionsThe necroptosis-related lncRNA signature and molecular clusters indicated superior predictive performance in prognosis and the immune microenvironment, which may also provide guidance to drug regimens for immunotherapy and provide novel insights into precision medicine.

Background: Despite several, there were many Breast Cancer (BC)- related deaths around the world. Searching for new analytical tools for BC detection at early stages is widely demanded. Aim: We aimed to evaluate Human Epididymal Protein 4 (HE4) clinical value in BC development and progression. Methods: HE4 serum levels were determined by ELISA in females with BC (n=120), benign breast disease (n=40) and healthy women (n=40). Receiver operating characteristic curve was applied for evaluation HE4 diagnostic power. Results: In BC patients, HE4 [5 (2-11.9) vs. 3.1 (1.8-5.4) and 1 (1- 3.5); P=0.022] was significantly higher than that in benign and healthy females, respectively. Compared to CEA and CA-15.3, it had superior diagnostic ability (AUC=0.783; P<0.0001; 73.3% sensitivity; 63.7% specificity). This ability increase when comparing BC patients to only healthy controls (AUC=0.862; 73.3% sensitivity; 77.5% specificity). Elevated HE4 levels were associated disease progression including multiple lesions, late stages, high grades, large size, lymph node invasion and non-luminal molecular subtypes. Spearman correlation analysis revealed that HE4 high levels significantly (P<0.05) correlated with tumor stage, histological grade and tumor size. Conclusions: HE4 is inexpensive, rapid, easy to perform and reliably BC biomarker. Moreover, its association with disease severity may support its potential role as prognostic BC marker.

Breast cancer is the tumor with the highest incidence in women worldwide. According to research, the poor prognosis of breast cancer is closely related to abnormal glucose metabolism in tumor cells. Changes in glucose metabolism in tumor cells are an important feature. When sufficient oxygen is available, cancer cells tend to undergo glycolysis rather than oxidative phosphorylation, which promotes rapid proliferation and invasion of tumor cells. As research deepens, targeting the glucose metabolism pathway of tumor cells is seen as a promising treatment. Non-coding RNAs (ncRNAs), a recent focus of research, are involved in the regulation of enzymes of glucose metabolism and related cancer signaling pathways in breast cancer cells. This article reviews the regulatory effect and mechanism of ncRNAs on glucose metabolism in breast cancer cells and provides new ideas for the treatment of breast cancer.
