Identifying Patients at Risk of Complete Nonresponse in the Outpatient Treatment of Depression

Abstract: Background: Although complete nonresponse in depression treatment is considered to be a major problem in clinical practice, research in this area is very limited. The objective of this preliminary study was to determine the frequency and predictors of complete nonresponse in different treatments for depression. “…Future investigations of increased power should include additional active treatment arms and clinical outcomes, in order to determine the specificity of these effects to particular treatments and affective symptoms. It is likely that mediators of treatment outcome are specific to treatment type [38]. Further, HA has exhibited associations with anxious as well as depressive symptoms [39]; the relative importance of different aspects of negative emotionality to serotonergic agents and to depression has yet to be established in a treatment context.…”

“…This cut-off was based on clinical consensus. On the one hand, it reflects slightly more improvement then the usual operationalization of <25% decrease for complete nonresponse [39,40,41,42]. On the other hand, it reflects a realistic approach to the expected change in the first phase of psychotherapy.…”

“…Further, the purpose of the science of clinimetrics is to provide a home for a number of clinical phenomena which do not find room in customary clinical taxonomy, such as type, severity and sequence of symptoms, rate of progression of illness (staging), well-being and distress [36]. In recent years, there have been several exemplifications of this approach in research on mood and anxiety disorders [38,39,40,41,42,43,44]. …”
