Idiopathic Membranous Nephropathy: Diagnostic and Therapeutic Challenges

Abstract: Background: In adults, membranous nephropathy (MN) is a major cause of nephrotic syndrome. While a majority of cases of MN are idiopathic, secondary forms can be seen in the setting of autoimmune disease, neoplasia, infection, and after being exposed to certain therapeutic agents. Both human immunodeficiency virus (HIV) and hepatitis C virus (HCV) infections could cause MN. However, the effect of coexisting HIV and HCV infection on the spectrum and progression of kidney disease as well as the effect of MN trea… Show more

“…Pentaacyltrehalose (PAT), [17] diacylated sulfo-glycolipidA c 2 SGL, [18] and polyacylated sulfoglycolipid SL-1 [19] are three representative examples of cell-wall lipids exclusively found in virulents trains of Mycobacterium tuberculosis.S ubstantial studies revealed that these lipids can not only potentially be used as biomarkers,b ut also are promising candidates for new tuberculosis vaccines. [17][18][19][20] The commons tructuralf eature of these lipids is encompassing at rehalose as ac entral carbohydrate, which is acylated with various long-chain fatty acids,t ypically polymethylated chiral deoxypropionate chains ( Figure 1). However,l ow availability of these lipids limited their furtherd evelopment as potential tuberculosis vaccines.…”

“…Mycolipenic acid 1,t he major acyl residue found in pentaacyltrehalose, has been shown to be ap otent inhibitor of leukocyte migration in vitro. [17,20] Mycolipenic acid 1 was first synthesized and characterized by Polgar et al in 1958 by al engthy route comprising ak inetic resolution. [21] Minnikin andc o-workers synthesized 1 as ar acemate in 1992.…”

“…Membranous nephropathy (MN) is the most common case of the nephrotic syndrome in adults in addition to the focal segmental glomerulosclerosis (FSGS), [ 1 , 2 ] about 80% of cases are idiopathic membranous nephropathy (IMN) and about 20% of cases are secondary membranous nephropathy (SMN). [ 3 , 4 ] Patients with MN who did not find the reason for the disease were called IMN.…”

“…To our knowledge, only two other cases of HIV-associated MN in which tissue PLA2R staining was performed are reported in the literature, one of which was negative [24]. The second reported case was PLA2R positive (tissue stain), in a patient with concomitant HCV infection [25].…”
