“Idiopathic” minimal change nephrotic syndrome: a podocyte mystery nears the end

Sumant S. Chugh,
Lionel C. Clement

Abstract: The discovery of Zinc Fingers and Homeoboxes (ZHX) transcriptional factors and upregulation of hypo-sialylated Angiopoietin-like 4 (ANGPTL4) in podocytes have been crucial in explaining the cardinal manifestations of human Minimal Change Nephrotic Syndrome (MCNS). Recently uncovered genomic defects upstream of ZHX2 induce a ZHX2 hypomorph state that makes podocytes inherently susceptible to mild cytokine storms resulting from a common cold. In ZHX2 hypomorph podocytes, ZHX proteins are redistributed away from … Show more

“…This fact makes scientific analysis challenging. However, due to an immunologically mediated pathophysiology, interaction with the complement system is at least conceivable [90]. Nevertheless, current data regarding complement activation do not extend beyond measuring complement cleavage products in patients' blood.…”

“…Сходные данные были получены и в нашем исследовании [24]. Обсуждается также роль "факторов проницаемости", оказывающих повреждающее действие на подоциты, в частности гипосиалилированного ангиопоэтинподобного белка 4 (ANGPTL4), гиперэкспрессия которого индуцируется ZHX факторами транскрипции [25].…”
