IgE allergy diagnostics and other relevant tests in allergy, a World Allergy Organization position paper

Abstract: Currently, testing for immunoglobulin E (IgE) sensitization is the cornerstone of diagnostic evaluation in suspected allergic conditions. This review provides a thorough and updated critical appraisal of the most frequently used diagnostic tests, both in vivo and in vitro. It discusses skin tests, challenges, and serological and cellular in vitro tests, and provides an overview of indications, advantages and disadvantages of each in conditions such as respiratory, food, venom, drug, and occupational allergy. “…Presently, the standard diagnostic methodologies targeting immunoglobulin biomarkers are largely based on antibodies. Thus, infection-generated IgG and IgM are detected from serum and plasma samples by sandwich ELISA or lateral flow immunoassays; the detection of the extraordinary IgG antibody expression caused by cancer and autoimmune disease is determined by sandwich immunoassays [176]; and conventional determinations of IgE also employ antibody-based sandwich ELISA, RAST (Radio Allergo-Sorbent Test in serum) or immunoblot tests [51].…”

“…In previous studies, the total IgE level was high among ACO patients, and similar to FeNO, this measure was considered as a biomarker for detecting eosinophilic in lammation in COPD patients [18,25]. However, these studies included patients with a history of asthma, atopic dermatitis, and allergic rhinitis that could have increased the total IgE level, regardless of COPD [26]. In this trial, an IgE cut-off value of 300 IU/mL was used in accordance with the reference range of our medical laboratory, which was higher than that used previously [18].…”

“…All the patients were ordered to suspend the medications that could interfere with the test results (antihistamines, thricycles antidepressants and tranquilizers, beta-blockers, corticosteroids) the necessary days for each medication. 22,23 The clinical results were analyzed using descriptive statistics with Statistical Package for the Social Sciences (SPSS Ver22). The results were expressed in absolute frequency and percentages, stander deviation for its analysis and frequency tables and graphs were made where study variables were included.…”
