“…More¬ over, we have found that the Carhart notch is reduced on the average by 7.3 db as a result of surgery; Juers reported similar changes in the notch following fenestration. 4 Although there is obvious correlation be¬ tween the Carhart notch and fenestral oto¬ sclerosis, our observations suggest that the notch bears no relation to the cochlear cap¬ sular changes seen in otosclerosis.…”

“…These conditions are usually associated with abnormalities of the middle ear sound transmission system, which act as confounding variables, making it difficult to ascertain precisely how much of the observed CHL is due to a thirdwindow effect of the lateral canal dehiscence (31). Of interest to this discussion is a report by Juers (35) showing improved thresholds for bone conduction after fenestration for otosclerosis in 28 patients; these improved thresholds may be the result of the fenestration acting as a third window. Ribaric et al (36,37) fenestrated the lateral semicircular canal in 5 bilaterally profound deaf individuals and found that all had improved hearing by bone conduction testing, although the air-conducted thresholds remained unchanged.…”
