Ileal Malignant Melanoma Causing Intussusception: Report of a Case

Abstract: Cutaneous malignant melanoma (MM) often metastasizes to the gastrointestinal (GI) tract; however, primary MM of the small intestine is a controversial diagnosis. We report the case of a 76-year-old woman found to have a primary MM in the ileum. After clinical evaluation, the radiological workup, which included magnetic resonance enteroclysis (MRE), revealed a large polypoid intraluminal tumor. She underwent laparotomy and the lesion was excised. Histological examination of the resected specimen revealed morpho… Show more

“…5 These include Meckel's diverticulum, the appendix, polyps, carcinoid tumors, submucosal hemorrhage resulting from Henoch-Schönlein purpura, non-Hodgkin's lymphoma, intestinal malignant melanoma, foreign bodies, fecaloids, intestinal duplications, and ectopic pancreas or HGM. [5][6][7] The recurrence of intussusceptions following either a surgical or nonsurgical reduction enhances the probability of congenital morphological abnormalities. [4][5][6] If the reduction involves an older child, a pathological lead point must be suspected and the possibility should be investigated during reduction.…”

“…Against this theory is the argument of spontaneous regression of a pre-or coexisting cutaneous malignant melanoma that can be as such overlooked. The concept of primary small bowel malignant melanoma remains controversial (6,7).…”

“…2 Treatment in metastatic melanotic intussusceptions with obstruction is palliative and involves resection and anastomosis of the involved segment of the gut. It relieves obstruction and also helps in the control of blood loss.…”
