Imaging diagnosis of accessory and cavitated uterine mass, a rare mullerian anomaly

Abstract: Accessory and Cavitated Uterine Mass (ACUM) is a rare form of developmental Mullerian anomaly seen in young females, which presents as chronic recurrent pelvic pain and severe dysmenorrhea. It is an accessory cavity lying within an otherwise normal uterus. It is lined by functional endometrium and surrounded by myometrium-like smooth muscle cells; hence, it bears striking macroscopic and microscopic resemblance to the uterus. Hysterosalpingography (HSG), Ultrasonography (USG), and Magnetic Resonance Imaging (M… Show more

“…Though the MRI results were misinterpreted by our radiologist, MRI is the imaging modality of choice as it is non-invasive and clearly shows the pelvic anatomy, cavitated mass with hemorrhagic contents, and the uterus, myometrium, and endomyometrial interface [2].…”

“…Because of improved imaging techniques and increased awareness, many cases of JCA are now being reported. These lesions are known by several names such as JCA, juvenile adenomyotic cyst, and recently, as an accessory and cavitated uterine mass [1][2][3]. Its exact etiology is unknown.…”

“…Uterus, fallopian tubes and ovaries are usually normal. Gubernaculum dysfunctions : These cases are typified by accessory and cavitated uterine masses (ACUMs) with an otherwise normal uterus [ 51 53 ]. HSG will show a normal endometrial cavity and 3D-US and especially MR allow the right diagnosis.…”

“…MRI can also confirm the presence of a cavitated mass with hemorrhagic contents. 2 It is also important to rule out other pathologies, including other Müllerian anomalies or cystic adenomyosis. In order to evaluate for other Müllerian anomalies, a hysterosalpingogram should be performed to confirm a normal uterine cavity with patent Fallopian tubes.…”

“…Since that time, *36 cases [2][3][4] have been reported as accessory uterine cavities or juvenile cystic adenomas. These lesions are all masses lined with endometrial glands and stroma, are surrounded by myometrium, and are noncommunicating with the normal endometrial cavity.…”
