ABSTRACT:Background:Calcifying pseudoneoplasm of the neuraxis (CAPNON) is a rare tumefactive lesion. CAPNONs can mimic calcified meningiomas at the skull base.Methods:Here, we report two cases of CAPNON and present a systematic review of the literature on skull base CAPNONs, to compare CAPNONs with calcified meningiomas.Results:Case 1: A 57-year-old man presented with right-sided lower cranial neuropathies and gait ataxia. He underwent a subtotal resection of a right cerebellopontine angle lesion, with significant improvement of his gait ataxia. However, his cranial neuropathies persisted. Pathological examination of the lesion was diagnostic of CAPNON, with the entrapped nerve fibers identified at the periphery of the lesion, correlating with the patient’s cranial neuropathy. Case 2: A 70-year-old man presented with progressive headache, gait difficulty, and cognitive impairment. He underwent a frontotemporal craniotomy for a near-total resection of his right basal frontal CAPNON. He remained neurologically stable 7 years after the initial resection without evidence of disease recurrence. We analyzed 24 reported CAPNONs at the skull base in our systematic review of the literature. Cranial neuropathies were present in 11 (45.8%) patients. Outcomes regarding cranial neuropathies were documented in six patients: two had sacrifice of the nerve function with surgical approaches and four had persistent cranial neuropathies.Conclusion:While CAPNON can radiologically and grossly mimic calcified meningiomas, they are two distinctly different pathologies. CAPNONs located at the skull base are commonly associated with cranial neuropathies, which may be difficult to reverse despite surgical intervention.

Lee et al 8 further reported that MRI enhancement does not depend on the presence of calcified tissue and that most of the tumor signal is enhanced even in fully calcified meningiomas.…”

Primary ectopic meningiomas, although widely reported in multiple sites of the body, are particularly rare in the pleura. Here, we report a 35-year-old asymptomatic woman who was found to have a large mass in the right pleural area on physical examination chest radiography. Chest CT scan showed a large irregular mass from the right second anterior costal pleura to the right supradiaphragm, in which calcified plaques of varying sizes were widely and heterogeneously distributed. The mass was connected to the pleura (anterior rib pleura, mediastinal pleura, diaphragmatic pleura) in a wide base, with oblique “Z” changes in the coronal view. After the contrast agent injection, the mass exhibited mild enhancement on both arterial and venous phase scans. Furthermore, a linear enhancement that was indicative of “pleural tail sign” changes in the pleura adjacent to the mass was observed. The disease was preoperatively misdiagnosed as malignant pleural mesothelioma, and the postoperative pathological diagnosis was right pleural meningioma (gritty type). Therefore, we carefully analyzed its imaging features and differential diagnosis by consulting relevant literature.

There are many types of neoplasms in or around the foramen of Luschka (FL), and definitive diagnosis in some cases requires knowledge of imaging findings. The uncommon and challenging neoplasms with FL involvement considered in this study are exophytic brainstem glioma, primary glioblastoma of the cerebellopontine angle (CPA), primary anaplastic ependymoma of the CPA, choroid plexus papilloma of the FL, solitary FL choroid plexus metastasis, extraskeletal myxoid chondrosarcoma of the jugular foramen, paraganglioma of the jugular foramen, exostosis of the jugular foramen, psammomatous meningioma in the lateral cerebellar medullary cistern, epidermoid tumor of the fourth ventricle, and a hypoglossal schwannoma. These neoplasms may have overlapping clinical and imaging features, but some have relatively distinct imaging features. Knowledge of the key clinical and magnetic resonance imaging features of these unusual lesions with FL involvement is important for radiologists to improve diagnostic ability and to assist the referring physician in the appropriate management of the patient.
