Imaging features of synovial chondromatosis of the temporomandibular joint: a report of 34 cases

Abstract: To investigate the imaging features of synovial chondromatosis of the temporomandibular joint (TMJ), which is a rare benign arthropathy with cartilaginous proliferation.MATERIALS AND METHODS: Computed tomography and magnetic resonance imaging examinations of 34 patients with histopathologically confirmed primary synovial chondromatosis of the TMJ were reviewed retrospectively. Imaging features including the lesion epicentre, destruction/sclerosis of surrounding bone, calcification, periosteal reaction, osteoph… Show more

“…15,16 The narrower joint compartment and smaller synovial membrane area in the lower compartment of TMJ may be to blame for the low prevalence rate of SC there. 17 A clinical sign of tumourlike diseases, including diffuse synovial giant cell tumours, synovial cysts, and SC, is preauricular edema. 18 It is challenging to distinguish SC from TMD or osteoarthritis because of the other vague symptoms.…”

“…18,19 The low prevalence rate of SC in the lower compartment of the TMJ might be due to the more narrow joint compartment and smaller area of the synovial membrane. 20,21 Preauricular swelling is a clinical symptom of tumor-like conditions such as diffuse tenosynovial giant cell tumor, synovial cysts, and SC. 22,23 Other nonspecific symptoms make it difficult to distinguish SC from TMD or osteoarthritis.…”

“…[44] Radiologic features of synovial chondromatosis include joint effusion, abnormal synovial thickening and enhancement suggestive of synovitis, adjacent bone erosion due to mechanical pressure, and calcified or noncalcified intra-articular loose bodies [Figure 8]. [41,42] In chronic disease, there may be imaging features of coexistent secondary osteoarthritis and asymmetric joint space narrowing. ere is potential for intracranial tumor extension due to its close proximity to skull base.…”

“…Synovial chondromatosis is a benign but locally aggressive disease characterized by chondroid metaplasia of the synovium, with subsequent formation of intra-articular loose bodies, causing destruction of the joint. [ 41 , 42 ] In the head and neck, it frequently involves the temporomandibular joint. Synovial chondromatosis can be divided into two subtypes – primary and secondary.…”
