Imaging-Guided Treatment of Meniscal Cysts

Abstract: Meniscal and parameniscal pathology is usually diagnosed through magnetic resonance imaging. The availability of high-frequency high-resolution linear transducers for sonographic detection of peripheral meniscal and parameniscal pathology has made not only the diagnosis but also the treatment of symptomatic parameniscal cysts a more accessible option. This case report illustrates the use of sonographically guided meniscal cyst aspiration and injection as an alternative to surgical treatment in the proper clini… Show more

“…Chang reported a case of a meniscal cyst aspirated and injected with steroid under ultrasound guidance which resulted in the "mild" improvement of symptoms at 6 months. 31 In a case series of 18 consecutive patients with symptomatic MRIconfirmed meniscal cysts aspirated under ultrasound guidance and injected with methylprednisolone acetate and bupivacaine, all patients experienced an initial resolution of all symptoms. At a mean final follow-up of 10 months, 10 of these 18 patients (56%) continued to report complete resolution of symptoms, 2 patients (12%) had only slight "twinges of pain," whereas 6 patients (33%) experienced a cyst recurrence.…”

“…Needle aspiration of meniscal cysts may also play a role when combined with other procedures. 26,31,32 Lu reported a case in which an anterior horn tear of the lateral meniscus was being addressed arthroscopically. 32 In this case, intraarticular cyst decompression through the meniscus would have resulted in a subtotal meniscectomy.…”

“…13). 51,52 Paralabral cysts of the shoulder and hip are similar lesions, often associated with underlying labral tears, and they may be painful or result in entrapment neuropathy. 53 These too can be aspirated and injected with steroids using imaging guidance.…”

“…23 While MRI remains the gold standard for diagnosis, meniscal cysts may also be diagnosed clinically when palpable or with the use of in-clinic musculoskeletal ultrasound in cases where MRI is not readily available or contraindicated. [24][25][26][27][28] Meniscal cysts can either be treated conservatively (aspiration of the cyst through a large bore needle followed by a corticosteroid injection; ultrasound assisted percutaneous needle decompression) or with surgical excision or decompression. [29][30][31] Open drainage with total meniscectomy was historically the preferred surgical technique for the treatment of meniscal cysts; however, this procedure has fallen out of favor with the advent of arthroscopic surgery and increased interest in meniscus preservation.…”
