Imaging in COVID-19-related myocardial injury

Abstract: Severe acute respiratory syndrome coronavirus 2 (SARS- CoV-2), previously named “2019 novel coronavirus” (2019-nCoV) is an emerging disease and a major public health issue. At the moment, little is known, except that its spread is on a steady upward trend. That is the reason why it was declared pandemic since March 11th, 2020. Respiratory symptoms dominate the clinical manifestations of the virus, but in a few patients also other organs are involved, such as their heart. This review article provides an overvie… Show more

“…Myocarditis-induced alterations may present with several LGE patterns, are typically localized at the sub-epicardial or intramural areas of LV and are frequently located in the basal to mid-inferolateral wall. LGE-positive patients are at increased risk of adverse cardiac events, while LGE-negative patients usually have an excellent prognosis, independently of their symptomatology [ 32 ]. The CMR diagnostic accuracy can increase significantly using the proposed updated LL criteria.…”

“…Cardiac MRI (cMRI) is the most sensitive imaging modality in detecting myocardial injury. Late gadolinium enhancement (LGE) has high sensitivity in detecting areas of myocardial fibrosis or necrosis and can also have prognostic value as it is associated with worse outcomes[ 113 ]. A typical cMRI protocol includes a short-axis CINE sequence for size and functional assessment, T1/T2 mapping for edema assessment, a delayed post-contrast scan for scar assessment and a T2 sequence.…”

“…Several reports describe clinical signs associated with COVID-19, with disease expression ranging from mild infection to severe acute respiratory distress. 2,3 In most patients, symptoms are mild. 4 However, certain patients, most notably elderly and those with comorbidities, can develop acute respiratory distress syndrome (ARDS) and other complications.…”
