Immediate Bilateral Breast Reconstruction With Implants and Inferolateral AlloDerm Slings

Abstract: To shorten the reconstructive process, improve results, and provide additional options for women seeking mastectomy, we used an acellular cryopreserved dermal matrix (AlloDerm) sling to reestablish the lower pole of the pectoralis major muscle. This technique creates a subpectoral-sub-AlloDerm pocket that completely encloses the breast implant. By tailoring the width of the AlloDerm, we can precisely control the degree of lower-pole fullness. This technique shortens or eliminates the need for tissue expansion … Show more

“…Complex patient selection and the requirement for preoperative CT (in DIEP flap surgery) also make autologous reconstructions a less attractive reconstructive technique35, 36, 37. ADMs were first reported in 200538. Increasing utilization of ADMs may have contributed to the increase in implant‐based reconstructions as their use obviates the need for total muscle coverage, which frequently requires the use of a tissue expander in a two‐stage procedure.…”

“…Biological matrices have been used in breast reconstruction since 2005 [10] and are now used by the majority of plastic For categorical variables, n (%) is presented. For comparison between the groups, Fisher's exact test (lowest one-sided p value multiplied by 2) was used for dichotomous variables.…”

“…The usage of matrices was first reported in secondary aesthetic breast surgery in 2001 [9] and in breast reconstruction in 2005 [10]. Reports have stated that the majority of plastic surgeons in the USA now use a matrix in implant-based breast reconstruction [1].…”

“…In the original report of Breuing as well as in five subsequent randomized studies the effectiveness and success of immediate reconstruction when using ADM were proven [38,41,42,[47][48][49]. In these retrospective studies, the overall incidence of complications was between 6,9% and 25%.…”
