Efficacy of acupuncture for lifestyle risk factors for stroke: A systematic review

BackgroundModifications to lifestyle risk factors for stroke may help prevent stroke events. This systematic review aimed to identify and summarise the evidence of acupuncture interventions for those people with lifestyle risk factors for stroke, including alcohol-dependence, smoking-dependence, hypertension, and obesity.MethodsMEDLINE, CINAHL/EBSCO, SCOPUS, and Cochrane Database were searched from January 1996 to December 2016. Only randomised controlled trials (RCTs) with empirical research findings were included. PRISMA guidelines were followed and risk of bias was assessed via the Cochrane Collaboration risk of bias assessment tool. The systematic review reported in this paper has been registered on the PROSPERO (#CRD42017060490).ResultsA total of 59 RCTs (5,650 participants) examining the use of acupuncture in treating lifestyle risk factors for stroke met the inclusion criteria. The seven RCTs focusing on alcohol-dependence showed substantial heterogeneity regarding intervention details. No evidence from meta-analysis has been found regarding post-intervention or long-term effect on blood pressure control for acupuncture compared to sham intervention. Relative to sham acupuncture, individuals receiving auricular acupressure for smoking-dependence reported lower numbers of consumed cigarettes per day (two RCTs, mean difference (MD) = -2.75 cigarettes/day; 95% confidence interval (CI) = -5.33, -0.17; p = 0.04). Compared to sham acupuncture those receiving acupuncture for obesity reported lower waist circumference (five RCTs, MD = -2.79 cm; 95% CI: -4.13, -1.46; p<0.001). Overall, only few trials were considered of low risk of bias for smoking-dependence and obesity, and as such none of the significant effects in favour of acupuncture interventions were robust against potential selection, performance, and detection bias.ConclusionsThis review found no convincing evidence for effects of acupuncture interventions for improving lifestyle risk factors for stroke.

Background: Hypertension is recognized as a major health problem throughout the world leading to various life-threatening heart diseases. The higher the level of blood pressure in the long run, the more likely there is a complication of hypertension. Slow breathing exercises are one of the non-pharmacological therapies that can be used to control or reduce blood pressure and strengthening breathing muscles. This systematic review is to analyze the effectiveness of slow breathing training in blood pressure in patients with hypertension.Methods: Literature search is through Scopus, Pubmed, Proquest, Science Direct, and EBSCO. with the last 5 years (2014-2019). Criteria for inclusion in this review are as follows: studies involving hypertension, ages 18 years and over, patients who have received or are still receiving slow breathing exercises. Keyword in this review was Slow Breathing, Blood Pressure, and Hypertension.Results: There are 10 journals chosen. Total respondents in this review were 1757 hypertensive patients who received slow respiratory therapy. Slow breathing training provides many benefits for hypertensive patients. Subjects with hypertension, pre-hypertension, and antihypertensive drugs, were given slow breathing intervention that is inhaling as much as ≤10 breaths/minute carried out for ≥5 minutes. This exercise is carried out at ≥3 days/week; total duration of intervention ≥4 weeks; Slow breathing training reduces blood pressure at rest, the average using this slow breathing exercise, blood pressure drops 10 -15 mmHg in systole and diastole. Overall, slow breathing exercises decreased SBP by -5.62 mmHg [-7.86, -3.38] and DBP by -2.97 mmHg [-4.28, -1.66].Conclusion: Slow breathing training provides many benefits for hypertensive patients. Especially for patients with isolated systolic hypertension. This non-pharmacological therapy can be carried out by individuals independently in controlling the breathing rate so that they can reach breathing speed both during inspiration and expiration. Slow breathing exercises can also help increase baroreflex sensitivity, then affect blood pressure reduction.

Content, Structure, and Delivery Characteristics of Yoga Interventions for Managing Hypertension: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

ObjectivesThis systematic review aimed to synthesize the content, structure, and delivery characteristics of effective yoga interventions used for managing hypertension and to compare these characteristics with ineffective interventions.Design and MethodThe JBI and the PRISMA guidelines were followed in this systematic review. RCTs conducted among hypertensive adults were included. RCTs reporting at least one of the major components of yoga (i.e., asana, pranayama, and dhyana and relaxation practices) and comparing them with no intervention or any intervention were eligible. Sixteen databases were searched for published and unpublished studies without any date and language restrictions till March 15, 2021.ResultsThe literature search yielded 13,130 records. 34 RCTs (evaluating 38 yoga interventions) met the inclusion criteria. Overall, included studies had low methodological quality mostly due to inadequate reporting. Yoga reduced SBP and DBP compared to a control intervention (MD −6.49 and −2.78; 95CI% −8.94– −4.04 and −4.11– −1.45, respectively). Eighteen, 14 and 20 interventions were effective in improving SBP, DBP, or either, respectively. 13 out of 20 effective interventions incorporated all the 3 major components of yoga and allocated similar durations to each component whereas ineffective interventions were more focused on the asana and duration of asana practice was longer. The most common duration and frequency of effective interventions were 45 min/session (in 5 interventions), 7 days/week (in 5 interventions), and 12 weeks (in 11 interventions) whereas the most common session frequency was 2 days a week (in 7 interventions) in ineffective interventions. Effective interventions were mostly center-based (in 15 interventions) and supervised (in 16 interventions) and this was similar with ineffective interventions.ConclusionDespite the low quality and heterogeneity of included studies, our findings suggest yoga interventions may effectively manage hypertension. The differences between the effective and ineffective interventions suggest that effective yoga interventions mostly incorporated asana, pranayama, and dhyana and relaxation practices and they had a balance between these three components and included regular practice. They were mostly delivered in a center and under supervision. Future studies should consider developing and evaluating an intervention for managing hypertension using the synthesized findings of the effective interventions in this review.Systematic Review Registration[PROSPERO], identifier [CRD42019139404].
